Submitting the Referral Form
We value the privacy and security of sensitive information. Do not email completed referral forms, as email is not a secure method for transmitting confidential information. Instead, mail your completed referral to the address at the bottom of the form.

Any individual aged 16 or older with a documented disability may be eligible for services from The Spectrum Network (TSN). Services provided include:
- Day Habilitation Services
- Job Coaching
- Job Development
- Home-Based Habilitation
MISSION STATEMENT: The Spectrum Network is committed to empowering individuals with cognitive disabilities, mental illness, and other barriers to independent living by supporting their integration into the workforce and community.
- Have identified vocational, educational, or health-related desired outcomes.
- Secure funding prior to the start of services.
- Be at least 16 years of age.
- Not pose a risk of harm to themselves or others.
- Have a diagnosis of one or more of the following:
- Intellectual Disability
- Mental Illness
- Developmental Disability
- Brain Injury
- Provide a Social Security Number (required for vocational services).
- Possess a valid picture identification card (required for vocational services).
- Be approved by TSN’s screening committee.
- The individual seeking services.
- Family members.
- Case Managers.
- Other professionals.
- Complete the organizational Referral Form. The individual seeking services must sign the form and express willingness to participate.
- Attend an initial intake interview, which will be scheduled by organizational staff.
- Submit the following documentation to expedite the referral process:
- A physical examination report completed within 60 days prior to admission.
- Psychological and/or psychiatric evaluation reports.
- Signed releases of confidential information forms.
- Other relevant documentation regarding the individual’s need for services.
- Collaborate with organizational staff to ensure funding is obtained.
- Organizational staff will conduct a screening to determine eligibility for services.
- The individual will be notified of the screening results with one of the following:
- A letter, email, or phone call confirming the start date if eligible.
- Recommendations for referrals to other appropriate agencies if deemed ineligible.
